Draft
In about a minute, PatientPilot builds a structured plan: active problems by body system, medicines, out-of-range results with charts of how they've changed over time, imaging, specialists, immunisations (recommended, funded and up to date), preventive-care gaps, guideline flags and a 3-monthly review plan.
How complete the draft is depends on how information is saved in your clinical software. -
Review
The nurse checks and refines the draft. The GP reviews it, discusses it with the patient and owns the final plan.
No printing and no scanning. The patient and GP sign by scanning a QR code on their phone, and admin staff link the saved, signed file to the patient's record.

Every chronic care patient will need a new plan
- GP Chronic Condition Management Plans (Medicare items 965 and 967) replaced GP management plans and team care arrangements on 1 July 2025.
- A plan can be prepared once every 12 months and reviewed every 3 months.
- The transition period ends on 30 June 2027. From 1 July 2027, patients need a GPCCMP for ongoing allied health access.
- Preparation is manual. A practice nurse typically spends 30 to 60 minutes per plan searching the record.
- Many of the same patients are also due a Domiciliary Medication Management Review (DMMR), Medicare item 900, with an accredited pharmacist. The planning visit is a natural time to refer, and the draft's medicine list gives the GP a head start.
What the draft covers
Each section is pulled from the record and laid out the way a GP expects to read a plan, ready for the nurse to check.

- Grouped by body system, so the whole picture is clear at a glance.

- The current list, with doses, ready to reconcile with the patient.
- Pathology over time
- Out-of-range results highlighted, with charts of key results like HbA1c, eGFR and LDL over the years against their targets.

- Key findings from reports on file, summarised.

- Who else is involved in the patient's care, from letters in the record.
- Immunisation
- Each vaccine shown as recommended or not, funded or not, and whether the patient is up to date. Screening that looks due is listed too.

The draft is only as good as the record it reads
PatientPilot drafts from what's saved in your clinical software. Results, immunisations, problems and letters recorded in their proper fields come through well. Plans per day are shown for an 8 to 10 hour nurse day (60 minutes per plan today gives 8 to 10 plans). Billing uses the GPCCMP fee of $156.55 at the July 2025 launch (indexed each year). Illustrative only: every plan needs a GP consultation and an eligible patient, so GP time, patient demand and Medicare requirements remain the limits. Revenue is an outcome of reaching more patients, not the goal.
